Forms Committee Report

The newsletter response from the Forms article was amazing! From the
article there were 15 requests for CD’s. These were mailed on October 9, 2008.

I received an email request from Franklin County Corrections for a forms CD;
it looks like we are expanding the ‘records’ wall. Due to the jail forms being fairly
limited on the CD, I asked that she send me any forms she could share. I haven’t
received any yet, but will follow-up with her as a reminder.

A request was received from Lisa Root, Quincy PD for any ACCESS SOP that has
been approved (accepted) during a State audit. I replied that we currently don’t
have any; however I plan to locate some reference documents.

The next newsletter article will be to request new forms.
