L.E.I.R.A. Training

When you are ready to sign-up for sessions please read the following directions carefully.
There are two options available to submit the form:
- Fill-in online and submit via email or
- Download the form to print, fill-in and mail.
- Adobe Reader required - preferred.
- Open the form by clicking here.
- Fill out all of the relevant fields using a PDF reader application (Adobe, Nitro, Foxit, etc.)
- Select the option on the bottom of the form to submit and pay with PayPal
- If you have a desktop email application (Outlook, Live Mail, or Mac Mail) the system should automatically create an email addressed to memberships@leirawa.org with the PDF form attached
- Alternate: If you do not have a desktop email application installed you will need to save the filled out PDF form to your desktop and submit the form via email manually to memberships@leirawa.org
- Open the form by clicking here.
- Fill in the form online
- Print and mail with your check to the address on the bottom of the form
- Allow 2-3 weeks for processing
